☐ Certification of Workers’ Compensation Insurance or Waiver

If you have employees, you will be required to provide a worker’s compensation insurance certificate. If not, you will be required to file a waiver with the Colorado Department of Labor and upload a copy of the waiver. ***NOTE: Paying someone with a 1099 does not make them a contractor. Colorado Department of Labor statute outlines the minimum criteria to consider when establishing a working relationship.
☐ Colorado Secretary of State Certificate of Good Standing
